This year, North Western Melbourne Primary Health Network commissioned and managed more than 350 contracts, and collaborated on hundreds more projects across our region, state and country.
We also supported more than 500 general practices and thousands of primary care providers through a period of significant reform.
This report contains a snapshot of our work in 2023–24, along with links to case studies, videos, outcomes and more.
It is a testament to the work of our care providers, staff, board of directors, and expert advisors including people with lived experience, their families and carers.
Dealing with personal ill health is challenging enough without also having to navigate a disconnected health system.
At every age, in every community, everyone has a right to equitable access to health care.
In health care, to say that a problem has been solved or a service gap filled is to misunderstand the nature of the issue.
At NWMPHN we believe that general practice is the heart of our health system.
This report provides the financial statements, board and auditor's reports for 2023-24, supporting NWMPHN to continue to improve health outcomes, strengthen primary health care and connect services across the system.
