Quality improvement (QI) is the basis of contemporary high-performing primary care. It includes team-based approaches, peer review, reflective practice and data analysis.
NWMPHN has an extensive QI program, which offers more than 500 practices tailored options including self-paced learning, 12-month plans, structured projects and more.

Eye health QI project
The Embedding Eye Health Preventative Care into Primary Care pilot project increased screening rates and detection of eye conditions and diseases for at-risk people.
Outcomes included more frequent in-practice assessments and stronger referral pathways.

Diabetes prevention QI project
The Risk Assessment, Recall and Referral project was a 12-month initiative that aimed to prevent the onset of cardiovascular disease and type 2 diabetes mellitus. NWMPHN commissioned 12 general practices to identify at-risk patients and provide them with lifestyle modification advice and referral to appropriate services.

Quality improvement program
Three-quarters of the general practices in our region participated in the Australian Government's QI Incentive program in 2023-24. Each practice is supported by a dedicated quality improvement program officer. Feedback shows that our support consistently rates as 'great' – our average net promoter score for 2023-24 was 67 per cent.
Self-paced QI tools
In addition to in-person support, NWMPHN has an extensive library of QI workbooks, templates, and other resources to support practices. In 2023-24, our QI tools were downloaded 869 times – helping practices make continuous, sustainable improvements, while supporting GPs to meet their continuous professional development requirements.
Heart failure QI project
The Managing Patients with Heart Failure in General Practice project improved the capacity of general practice to better assess people at risk of heart disease and manage patients with heart failure.
Project outcomes included improved identification of heart failure, increased clinician confidence, and better access to nurse practitioners and advanced practice nurses.
