NWMPHN is committed to working with funders, service providers and people with lived experience to design and commission effective multidisciplinary team care projects – and to evaluate the outcomes.

Early interventions for healthy ageing
The Australian Government funds NWMPHN to support older people to live at home for as long as possible, reducing pressure on hospitals, aged care and other health services.
To do this, we commissioned 10 general practices to identify older people who have chronic health conditions, and provide them with free, multidisciplinary care and early interventions to promote healthy ageing.

Pharmacists in general practice
NWMPHN funded the Pharmaceutical Society of Australia to implement the Pharmacists in General Practice program in the region.
The program brings together elements of workforce capacity building, chronic disease prevention and management, and quality use of medicines principles. It improves medication use by patients and GPs, boosting health literacy and medicines adherence.
Carer wellbeing and connection – In Touch initiative
NWMPHN engaged Carers Victoria to develop and deliver an innovative approach to improve social connectedness for people experiencing poor health outcomes in Hume and Wyndham.
With a focus on carers of people with disability, mental illness, chronic health issues or age-related conditions, the program provides group discussions, individual planning and support, and follow-up care.
Youth Projects – Saturday clinic and after-hours assertive outreach programs
NWMPHN commissions Youth Projects to deliver location-based after-hours primary health services for people experiencing or at risk of homelessness.
Youth Projects has a strong multidisciplinary team with skills and expertise in addressing a broad range of health issues. This skill set is the ideal match for the diverse and complex physical, mental and social health needs of the people who utilise the service.
Connecting the dots
We acknowledge the people of the Kulin nations as the Traditional Custodians of the land on which our work in the community takes place. We pay our respects to their Elders past and present.
We also recognise, respect and affirm the central role played in our work by people with lived experience, their families and carers.
